I love the tear drop and the eyes but the background doesn't help too much in this picture. it's too close to the baby's skin and has nothing to guide us or whatever. There is enough shading in the background to make it seem important, you even have depth in darks and lights, but there is really nothing there. Your proportions are dead on for that head, I always find babies and their subtle depressions in their skin difficult to draw properly.
